Prophylactic antibiotics following the treatment of overactive bladder in women with Botox for the reduction of postoperative bladder infection
Overactive bladder affects up to 40% of women. This condition includes symptoms of feeling the need to empty the bladder urgently, urinating frequently, waking at night to empty the bladder, and sometimes urinary leakage. These symptoms can be disruptive to day-to-day living. Oral medications used to treat this condition are commonly stopped because they are not effective or they cause side effects. When treatment with oral medications is not successful, injection of Botox into the bladder is a procedure used to treat overactive bladder. Botox is a medication used in multiple medical specialties to paralyze muscles in different parts of the body. In the bladder specifically, it paralyzes muscle in the bladder wall which helps reduce symptoms caused by overactivity of these muscles. A common complication of bladder Botox is bladder infection following the procedure. Bladder infections can cause irritating symptoms and can lead to more serious illness including kidney infection or sepsis. Preventative antibiotics given at the time of bladder Botox for the reduction of bladder infection following the procedure have not been well studied. The main goal of our study is to determine if preventative antibiotics at the time of bladder Botox injections for the treatment of overactive bladder in women reduces postoperative bladder infection. We are proposing a study which will randomize participants into two groups - one receiving antibiotics and the other receiving placebo pills for three days following the procedure. The primary outcome evaluated will be the difference in postoperative bladder infection between the two groups. We will also investigate differences in reported side effects between the two groups possibly related to antibiotic use. Follow-up will be over six weeks following the procedure. There is growing concern in the medical community over antibiotic resistant bacteria caused by improper use of antibiotics. Findings from our study may help guide clinicians on the proper use of antibiotics for a specific preventative use. We hope the results of our study can be used to improve safety and outcomes for a Urogynecologic procedure used to treat a common bladder condition in women.
